Transitie

Patiënten die onder behandeling zijn van een kinderarts-immunoloog/reumatoloog/infectioloog worden vanaf hun 18e jaar vervolgd op speciale transitiepoli’s. Hier zijn zowel de behandelend kinderarts als de internist-immunoloog/nfectioloog aanwezig, zodat de overgang van zorg soepel verloopt. De transitie en het moment van overgang worden al aangekondigd tijdens onze tweewekelijkse immunologische besprekingen, zodat de overgang naar volwassenenzorg zonder problemen kan verlopen. 

Patients treated by a pediatric immunologist, rheumatologist, or infectious disease specialist are followed from the age of 18 at special transition clinics. Both the treating pediatrician and the internist-immunologist or infectious disease specialist are present here to ensure a smooth transition of care. The transition and the timing of the transfer are already announced during our bi-weekly immunological meetings so that the move to adult care can proceed without any issues.
